PhD position summary/title: PhD Project Title: Balancing Supply and Demand: Developing a Net Zero Energy Framework for Difficult-to-Retrofit Buildings in Nottinghamshire

Nottinghamshire County Council (NottsCC) recently launched its 2024 Net Zero Framework, which sets out how the council will become net zero by 2030. One of the key priorities is to reduce its direct emissions by improving the energy efficiency of its buildings. Whilst NottsCC has reduced emissions from its buildings by 59% since 2014-15, energy use still accounts for 31% of its total baseline emissions. Budget cuts and inflation mean that reducing energy use to a minimum is essential not only for reaching net zero but to ensure the future financial sustainability of the council.

A key area of concern for NottsCC is how to improve the energy efficiency and heating controls of some of its poorest-performing building stock. Between the 1950s and 1980s, a consortium of East Midlands local authorities used a construction method called CLASP to build low-cost, lightweight buildings. Whilst revolutionary at the time, they contain Asbestos, which is a serious public health issue. Nottinghamshire has the highest number of CLASP buildings in the UK. They are poorly insulated and expensive to heat, but difficult to modify due to the asbestos. There are too many buildings in use with CLASP Construction for them to be replaced in one go, thus a strategy is needed to identify those which can benefit from energy efficiency performance improvements.

This PhD project aims to develop a framework to improve the energy efficiency of difficult-to-retrofit buildings within Nottinghamshire and raise awareness of NottsCC’s Net Zero activities. Different classes of CLASP buildings within the NottsCC portfolio will be reviewed to identify opportunities for energy efficiency interventions. Furthermore, an evidence-based guide on heating control systems for different building types will be developed to inform future heating system upgrades and new builds. The project will work with Inspire to raise awareness about NottsCC’s Net Zero work and to identify how local communities can contribute to the Net Zero journey.

PhD position summary/title: PhD Studentship- Health economic evaluation of community acquired pneumonia in the UK

Pneumonia is a common illness that results in over 100,000 hospital admissions per year in the UK. Following hospital treatment for pneumonia, 55% of patients visit their GP within 30 days of discharge from hospital while 15% are readmitted to hospital within 30 days of discharge. Despite the high morbidity experienced and described by patients, there are few interventions that are currently in place to support recovery.  To determine how to best intervene in a cost-effective manner, up to date cost estimates and quality of life measures for patients who have pneumonia treated both in the community and in hospital are required.

The first stage of the project will be a study of the economic burden of community acquired pneumonia within the UK. The student will analyse large datasets such as the Clinical Practice Research Datalink (CPRD) and Hospital Episode Statistics to identify activity related to the treatment of community acquired pneumonia. This will require identifying relevant treatments and their associated Health Resource Group (HRG) codes to link hospital care provided with NHS Reference Costs, as well as linking any primary care activity with Unit Costs of Health and Social Care, and British National Formulae (BNF). Costs in primary and secondary care will be combined to estimate total healthcare cost and total healthcare burden to the NHS, and sub-group analyses by age, ethnicity, and index of multiple deprivation will be performed. The second stage of the study will involve prospectively collected EQ-5D-5L data from a cohort of patients to determine the impact of community acquired pneumonia that requires hospitalisation  on the quality of life of patients. Data will be collected at baseline, three months, six months, and 12 months from diagnosis. Questionnaires will be collected as part of routine care through research active nurses, with data passed to the student The student will need to clean and prepare the datasets, and score the EQ-5D-5L using UK specific value sets. Missing data will be controlled for using multiple imputation, with sub-group analyses by age, ethnicity, and index of multiple deprivation.

PhD position summary/title: PhD studentship: Identifying repurposed treatments for rare lung diseases

This studentship, based at the University of Nottingham is aligned to the LifeArc. Translational Centre for Rare Respiratory Diseases. The Centre funded at £9.5M over five years is a national collaborative grouping that will change the research landscape for people affected by rare lung diseases. The studentship will focus upon modelling and drug discovery in one of the Centre’s exemplar diseases, lymphangioleiomyomatosis (LAM). LAM is a rare disease which almost exclusively affects women and is caused by somatic or germline mutations in the tuberous sclerosis complex (TSC) genes. LAM is incurable, causes lung cysts, lymphatic abnormalities and renal tumours with patients suffering progressive breathlessness and lung collapse which may lead to respiratory failure and death. Lung, lymphatic and renal problems result from the accumulation of LAM cells, a clone of cells with mutations in the TSC genes resulting in activation of the mechanistic target of rapamycin (mTOR) resulting in uncontrolled proliferation, migration and the ability to survive in adverse environments. The mTOR inhibitor rapamycin is licenced for LAM, reduces decline in lung function but is not curative and new treatments are needed. The student will examine the mechanisms of lung damage in LAM; using and developing complex tissue culture models for applications in drug repurposing in collaboration with other members of the LifeArc Centre.

PhD position summary/title: PhD Studentship: Improving health and wellbeing in Nottingham through improved understanding of the link between air pollution and health.

The harmful effects of air pollution on health are known, but the improvements in air quality reported UK-wide are leading to better health outcomes as expected. This may well be because although standard measures of air pollution such as fine particulate matter and nitrogen dioxide are declining, the composition of the air we breathe and the timing of peak levels of pollution (episodes) is changing in response to changes in both emissions and climate. In the health sector, emergency respiratory hospital admissions due to ozone are projected to increase, presenting a major concern. There is thus an urgency to improve our understanding of the relationship between air quality and health data, particularly at the local scale, where impacts on individuals, communities and health services will be most evident.

Through a collaboration with Nottingham City Council, a comprehensive local network of air quality monitors allows us to track, in real-time, a number of key air pollutants alongside temperature and traffic levels.  Used in conjunction with an air pollution model, this data will be used by this project to assess pollution concentrations across the city in unprecedented detail. Health data from Nottingham University Hospitals Trust will be used to track acute presentations of illness and link this to air pollution data.

This PhD research project will combine air quality and health data, using Nottingham as a case study. This work has the potential to inform new approaches to air quality targets in the UK and beyond and could provide a step change in preventative advice to communities most at risk of acute air pollution-linked health conditions. Ultimately, the outcomes of this research could help reduce morbidity and local health service pressures and have wider economic benefits.
